RN Diabetes Patient Educator

The Diabetes Patient Education Coordinator is responsible for the provision of consistent, evidence based education to this specific patient population either directly or by disseminating information and teaching materials to RNs who will then teach the patient themselves. Demonstrates responsibility for the overall fiscal management of the Diabetes patient teaching program, conducts and manages performance improvement activities, develops and monitors relevant teaching policies and procedures. Develops and provides coordination of disease specific patient teaching plans. Is responsible and accountable for the provision of disease specific patient teaching plans. Exhibits a variety of clinical and interpersonal skills. Interacts effectively with patients, families, referral sources, physicians, hospital departments, and other members of the hospital health care team. Works in close collaboration with all members of the health care team.
-Reports to Director of Nursing with dotted line to service line administrators
-Supervises: patient education provided by other care givers
-Responsible for Budget
-Financial Management Prepares and manages yearly teaching materials budget. Identifies and explains budget variances to the Administrator. Prepares monthly patient teaching report card.
Monitors expenses related to patient education and assures adherence to policies by staff.
-Patient Care Management - Develops and implements patient education for specific disease, Diabetes, including protocol development in collaboration with the patient education team. Coordinates each patient education program activity conducted within the hospital for this specific patient population. Maintains appropriate records. Monitors and reports the appropriate metrics to OSQC. Disseminates best practice information for application in patient education, including published findings from research journals. Presents this information to nursing and medical staff. Consistently meets and complies with regulatory standards and appropriate credentialing bodies. Actively participates in the patient education committee. Creates a culture of service excellence that ensures patient, family and coworker satisfaction:
a) Demonstrates consistent ability/competence in patient care areas.
b) Provides direct patient education provided by staff.
c) Reduces conflict and prevents undue anxiety in patients, families and all members of team.
Consistently establishes professional standards for quality, legal and ethical practice. Promotes and maintains patients rights and responsibilities. Maintains professional image in dress, speech and actions.
Establishes collaborative relationships with physicians and members of the team to achieve ollaborative education plans and outcomes. Creates and maintains an environment that promotes patient safety.
Complies with established nursing standards. Documents and submits follow up problem solving to the Director of Nursing involving patient complaints, medication errors, and patient incidents and other concerns related to patient education. Exhibits skilled performance in managing stress. Acts as a liaison between patients, families, and health care providers. Supports the creation of team work and team spirit both interdepartmentally and intradepartmentally. Provides structure that facilitates emotional and situational support for patients and families.
Education, training, experience:
Must be a Registered Nurse with active licensure in the Commonwealth of Virginia or Compact State, BSN required, MSN preferred.
Basic Life Support Certification. Obtain Certified Diabetes Educator Certification within two years of hire or accepted position.
Minimum of 2 years of clinical experience.
